Williamson County and Cities Health District Feedback Survey

All community members living and/or working in Williamson County are invited to participate in the Williamson County and Cities Health District 2022 Survey.

This survey will ask about your thoughts on the services provided by Williamson County and Cities Health District. The information we collect will inform how Williamson County and Cities Health District can best serve and promote the health of the communities in Williamson County.

The survey will be open from May 16 to June 24, 2022, and is available in English, online, or on paper. All respondents will remain anonymous. At the end of this survey, you will be able to enter a drawing for a $100 gift card.
